Hüft-Spacer
(Synicem) als temporärer Platzhalter bei infizierter Hüftendoprothese. Wenn die Infektion ausgeheilt ist, wird wieder eine dauerhafte Hüftendoprothese implantiert.
Röntgenbild
eines rechten Hüftgelenks mit zementfreier Endoprothese (Rechteckschaft und konische Schraubpfanne): Konzentrische Einstellung des Metallkopfs im Polyethylen-Inlay der Pfanne. Ausgeprägte heterotope periartikuläre Verknöcherung und massive Osteolyse in der Umgebung des Prothesenschafts. Bei operativer Revision massive Metallose.

periprothetische Ossifikation: links postoperativ, Mitte und rechts nach 5 Monaten.
Girdlestone-Situation
nach Ausbau einer infizierten Totalendoprothese des linken Hüftgelenks. Zur Zemententfernung wurde das obere Femurende gefenstert. Der Kortikalisspan ist mit drei Cerclagen refixiert. Im Pfannenlager und in der Markhöhle des Femurs sind PMMA-Ketten eingelegt.
Bruch des
Prothesenkopfes bei Hüft-Totalendoprothese: Linkes Bild mit intakter Prothese, aber schon reichlich periarthikuläre Ossifikationen. Rechtes Bild Fragmentierung des Prothesenkopfes. Die Ossifikationen haben auch zugenommen.
Hüftendoprothese
mit defektem Inlay, zumindest disloziert, mutmaßlich gebrochen. Der Prothesenkopf ist deutlich nach kranial lateral dezentriert.
Complications of total hip arthroplasty are common and it is essential for the radiologist to be aware of them in the assessment of radiographs of total hip replacements. Complications are many and can occur at various time intervals following the initial surgery:
- aseptic loosening: considered to be the most common indication for revision surgery
- infection of hip prostheses
- particle disease / histiocytic reaction / aggressive granulomatosis
- fracture
- periprosthetic fracture
- cement fracture
- stem fracture
- dislocation (~5% of all replacements )
- dislocation of femoral head out of the cup: component dissociation
- dislocation of the entire acetabular cup
- dislocation of stem
- malpositioning
- creep: can lead to protrusio acetabuli
- intrapelvic cup migration
- polyethylene wear eccentric position of the femoral head component within the acetabular component
- heterotopic bone formation
- reaction to metal
- pseudobursae formation
- abductor muscle/tendon dysfunction e.g. tears
- stress shielding
- trochanteric bursitis
- revision total hip arthroplasty
- general post-orthopedic surgical complications
- hemorrhage
- wound complications e.g. dehiscence
- thromboembolism, i.e. deep vein thrombosis and pulmonary embolism
- vascular injury
- neurological deficit
- death
A standardized list of complications post total hip arthroplasty was developed by consensus by The Hip Society. This enables consistent standardized reporting of complications .
