The differential diagnosis for extratesticular cystic lesions includes:

  • hydrocele
  • epididymal cyst
  • spermatocele
  • hematoma
  • haematocele
  • loop of bowel from an inguinal hernia
  • abscess
  • pyocele
  • post-vasectomy
  • varicocele

    • Very rarely, a scrotal mesothelioma may present as a cystic mass.
