Intrathoracic sleeve migration

Intrathoracic sleeve migration is a rare complication following a sleeve gastrectomy.

Pathology 

Several factors have been postulated, including

  • gastric narrowing
  • progressive enlargement of the esophageal hiatal orifice
  • division of natural attachments such as the phreno-esophageal membrane
  • removal of gastric fundus in close proximity to the angle of His
  • postoperative weight loss
  • decrease in the visceral fat
