with vomiting. AXR AP (above left) shows a distended air-filled stomach which on the AXR upright (above right) has a large air-fluid level within it. AP image from an upper GI (below) shows that by following the course of the feeding tube the stomach appears to be upside down with the pylorus of the stomach located superiorly to the gastroesophageal junction and the tip of the feeding tube to be in the second part of the duodenum.The diagnosis was mesentero-axial gastric volvulus.

chronic gastric volvulus. Large-sized air pocket in the chest, without evidence of the typical gastric air pocket in the left upper quadrant.

chronic gastric volvulus. Dislocated stomach into the thorax, rotated in the mesenteric axis, resulting in alpha configuration. The gastric antrum is located above the fundus. The barium filling is inverted, the inferior part fills earlier than the superior.

chronic gastric volvulus. CT made 4 years earlier in a symptomatic period was interpreted as having a large hiatal hernia, with gastric dislocation into the thorax and moderate dilatation. A nasogastric tube was adequately placed in the gastric lumen.
