Morel laminar sclerosis

Morel laminar sclerosis is characterized by cortical laminar necrosis and gliosis, especially affecting the lateral frontal cortex.

Pathology

Postmortem neuropathologic studies in patients with Marchiafava-Bignami disease have described cortical laminar necrosis and gliosis, mainly in the third layer of the cerebral cortex. It is probably associated and secondary to the more typical callosal lesions of Marchiafava-Bignami disease.

Although being a rare finding on imaging, Morel laminar sclerosis is seen mainly in the lateral-frontal cortices, showing typical signs of cortical laminar necrosis on MRI:

  • T2/FLAIR: cortical hyperintensity
  • DWI/ADC: high signal seen on DWI with corresponding low ADC signal
