Humpback deformity of the scaphoid results from volar angulation of the proximal and distal poles of the scaphoid in the setting of scaphoid fracture through the waist . The dorsal component forms a 'humpback' which can be palpated.
It is important to identify as it can result in a progressive collapse of the scaphoid with non-union, and even if an eventual union is achieved, destabilisation of the wrist . It is commonly associated with dorsal intercalated segment instability (DISI).
As such, these fractures are managed operatively with internal fixation with or without bone grafting.
