with right hip pain for 2 weeksAP radiograph of the right femur was unremarkable. Coronal STIR MRI of the right hip and femur was unremarkable aside from incidental bright pelvic free fluid noted just above the bladder. Coronal 2D reconstruction from a CT with IV and oral contrast of the abdomen shows a large complex fluid collection containing air and a central calcification in the right lower quadrant which was lying next to the psoas muscle.The diagnosis was perforated appendicitis with an abscess containing an appendicolith.

Axial CT with contrast of the abdomen (above) shows on top of the right psoas muscle and right iliac vessels in the right lower quadrant a dilated, fluid-filled tubular structure containing a calcification . Coronal CT (below) shows two calcifications within the tubular structure which is just medial to the cecum and surrounded by inflammatory changes.The diagnosis was acute appendicitis with multiple appendicoliths. Pathological examination additionally revealed a carcinoid tumor of the appendix.

An appendicolith is a calcified deposit within the appendix. They are present in a large number of children with acute appendicitis and may be an incidental finding on an abdominal radiograph or CT. Incidence may be increased among patients with a retrocecal appendix. Overall they are seen in 10% of patients, with 90% subsequently going on to develop appendicitis .

Pathology

Although the cause of appendicolith formation is uncertain, some case reports have mentioned an ingested foreign body or a dislodged gallstone eroding through the gallbladder as possible etiologies.

Radiographic features

If identified on ultrasound, an appendicolith will appear cast an acoustic shadow. On abdominal radiograph or CT, a high attenuation stone may be seen in the right iliac fossa. CT is more sensitive than plain film. Up to 25% show laminated calcification.
