The differential of a fat containing solitary pulmonary nodule is very narrow.
In a well circumscribed smooth or lobulated mass (especially if it has been largely stable in size over time) presence of fat is essentially pathognomonic of a pulmonary hamartoma, and usually not further assessment is required.

- pulmonary hamartoma: by far the commonest
- lipoma
- myelolipoma of lung
- lipoid pneumonia
- metastatic disease
