Fetal liver and splenic enlargement

Fetal hepatosplenomegaly refers to the combined enlargement of both the fetal liver and the fetal spleen.

Pathology

This can occur from a number of pathologies, including:

• transient abnormal myelopoiesis (TAM)  associated with trisomy 21
• in association with fetal hydrops
• in utero infections
  • fetal parvovirus B19 infection
  • fetal rubella infection
  • fetal toxoplasmosis
  • fetal cytomegalovirus infection
• fetal hypothyroidism
• due an underlying syndromic association
  • Beckwith-Wiedemann syndrome 
  • Zellweger syndrome 
• ​storage disease
  • Gaucher disease
  • Niemann-Pick disease
