Fat containing breast lesions generally have some radiolucent component on mammography.

Pathology

They are generally classified at BIRADS II lesions.

Common

• breast lipoma
• breast hamartoma
• fat necrosis within the breast/oil cyst
• intramammary lymph node: classically has a central fatty hilum
• galactocoele

Uncommon

• steatocystoma multiplex of the breast
• angiomyolipoma of the breast : very rare lesion
• well differentiated liposarcoma of breast

Note

• complex sclerosing lesion: 
  • looks like breast carcinoma, but often has a lucent center; usually 1-3 cm; at <1 cm these lesions have the same histology but are called radial scars and are not usually mammographically seen
