iliopsoas bursitis

Iliopsoas bursitis usually presents as non-specific anterior hip pain and can be due to a number of causes, the 3 main causes being acute trauma, overuse injuries, and rheumatoid arthritis.

Pathology

Iliopsoas bursitis can occur primarily, e.g. secondary to a snapping iliopsoas tendon or post hip arthroplasty. The iliopsoas bursa frequently communicates with the hip joint and intra-articular pathology (e.g. osteoarthritis, rheumatoid arthritis) can result in an iliopsoas bursitis .

Other causes include trauma, osteoarthritis, avascular necrosis, synovial chondromatosis, pigmented villonodular synovitis, gout, and pyogenic infection.

Radiographic features

Ultrasound
  • well-defined thin-walled fluid collection along the line of the iliopsoas tendon
  • contents can be homogeneous or heterogeneous  
  • may be superior to other modalities in showing a septations .
MRI
  • often shows a distended bursa
  • may be superior to other modalities in showing a communication between the bursa and the hip joint
