Bat wing or butterfly pulmonary opacities refer to a pattern of bilateral perihilar shadowing. It is classically described on a frontal chest radiograph but can also refer to appearances on chest CT .
Differential diagnosis
Bat wing pulmonary opacities can be caused by:
- pulmonary edema (especially cardiogenic)
- pneumonia
- inhalation injury
- noxious gas
- liquid
- pulmonary alveolar proteinosis
- pulmonary hemorrhage (e.g. Goodpasture syndrome)
- lymphoma/leukemia
- bronchoalveolar carcinoma
