Pachymeningeal enhancement
Magnetresonanztomographie
des Kopfes (T1w mit Kontrastmittel, coronar) bei Liquorunterdrucksyndrom nach PDA. Typisch ist die starke Signalintensität der Meningen nach Kontrastmittel.
Spontaneous
intracranial hypotension with spinal myelography imaging findings.. Unenhanced (a) and contrast-enhanced (b) axial T1-weighted images reveal diffuse pachymeningeal enhancement (arrows) and the “venous distension sign” (curved arrow). Also, note the presence of small bilateral subdural fluid collections (arrows).

Pachymeningeal enhancement, also known as dura-arachnoid enhancement , refers to a dural and outer layer of arachnoid pattern of enhancement seen following contrast administration and may occur in the conditions listed below:
- infection
- intracranial tumor
- metastases (meningeal carcinomatosis-pachymeningeal carcinomatosis)
- intracranial hypotension
- postoperative states
- idiopathic pachymeningitis
- cerebral venous thrombosis
- neurosarcoidosis
- extramedullary hematopoiesis
- rheumatoid arthritis
- granulomatosis with polyangiitis
Terminology
It is important to note that the thin arachnoid membrane is attached to the inner surface of the dura mater, and so the pachymeningeal pattern of enhancement can also be described as a dura-arachnoid enhancement .
On the other hand, the enhancement of the brain surface is also called as pial or pial-arachnoid enhancement, often referred to as leptomeningeal enhancement .
