Bilateral megalencephaly refers to megalencephaly affecting both cerebral hemispheres.

The diagnosis may be suggested when the HC:AC ratio is above the 99percentile without evidence of IUGR (i.e low AC), fetal hydrocephalus or an intracranial mass.
