- increased pulmonary blood flow
- total anomalous pulmonary venous return (TAPVR)
- pregnancy
- high output cardiac failure
- thyrotoxicosis
- anaemia
- Marfan syndrome
- idiopathic dilatation of the pulmonary trunk: diagnosis of exclusion
- mimics
- enlarged left atrial appendage
- left hilar/mediastinal mass
