The differential diagnosis for unilateral testicular lesions is wide-ranging.
Neoplastic

- non-seminomatous germ cell tumors:
- testicular teratoma
- testicular epidermoid (teratoma with ectodermal elements only)
- testicular choriocarcinoma
- testicular embryonal cell carcinoma
- testicular yolk sac tumor (endodermal sinus tumor)
- testicular mixed germ cell tumor
Uncommon

- metastasis to testis
Non-neoplastic

- focal orchitis
- focal intratesticular hemorrhage
- ischemia/infarction (e.g. testicular torsion)
