paratesticular lesions
Paratesticular lesions have a long list of differential diagnoses:
- epididymal cyst (most common epididymal mass)
- adenomatoid tumor (most common epididymal tumor)
- scrotal tunica cysts
- hydrocele
- scrotal haematocele
- varicocele
- scrotolith (scrotal pearl)
- inguinal herniation
- polyorchidism (supernumerary testes)
- spermatocele
- sperm cell granuloma
- scrotal lipoma (most common paratesticular neoplasm, usually arising from spermatic cord; seen in about 22% of patients who had hernia repair)
- scrotal leiomyosarcoma
- fibrous pseudotumor of the scrotum
- papillary cystadenoma (of epididymis): most common epididymal mass on those with von Hippel Lindau disease (vHL)
- testicular sarcoidosis
- tuberculous epididymo-orchitis/epididymitis
- spermatic cord
- scrotal hemangioma: is often hypervascular on color Doppler, unlike other benign paratesticular lesions that usually show little vascularity
