- giant cell tumor of bone: 2 most common primary sacral tumor
- aneurysmal bone cyst
- osteoid osteoma
- osteoblastoma: accounts for ~17% of spinal osteoblastoma(s)
- cavernous hemangioma of sacrum
- malignant
- neural tumors arising in sacral canal
- sacral ependymoma
- sacral schwannoma
- sacral neurofibroma
- malignant peripheral nerve sheath tumor
- pre-sacral tumors
- germ cell tumors
- sacrococcygeal teratoma
- sacrococcygeal yolk sac tumor
- germ cell tumors
- metastases to the sacrum

- spinal dysraphism
- sacral meningocele
- sacral myelomeningocele
- sacral lipomyelomeningocele
